Appointments
Please call our office to make an appointment. If you are unable to keep your appointment, please call us in advance so we may use your time to accommodate other patients in need of care. New patients considering surgery should bring in any past medical records pertaining to their orthopaedic condition, such as original X-rays, Imaging Reports, (MRI/Bone Scans/CT-Scans) and/or Operative Reports, so that the doctor can properly evaluate you. We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we are late due to emergencies or hospital surgery.Forms

Fees and Payments
We make every effort to decrease the cost of your medical care. Therefore, we request payment for all office services at the time they are rendered. We accept cash, checks, MasterCard, Visa, American Express, and Discover for your convenience. If we are a participating provider of your insurance company, we will bill them. HMO patients will be responsible for obtaining their referral/authorization, in advance, from their Primary Care Physician prior to their scheduled visit.Prescriptions and Renewals
Please request all prescription and authorization for renewals during regular office hours. To expedite your request, your pharmacy may fax the refill request directly to our office.
